Outreach Services

Who do we help?

Wintringham’s Outreach Services help men and women aged 50 years and over who:

  • are homeless or at risk of homelessness
  • may need assistance with managing tasks of daily living
  • receive little or no support from family or friends
  • may be socially and/or financially disadvantaged.

What help is available?

Outreach Workers are professionally trained and aim to help you make the changes you want in your life. This is achieved through providing information, support, referral to services and, importantly, advocacy.

The services offered may be provided by Wintringham and/or a range of other service providers: 

  • assessing a client who has nowhere to live and assisting them to access crisis or transitional housing services
  • referring a client into long-term community or public housing and arranging the appropriate support services for that client
  • providing advocacy to clients to ensure they access appropriate services to which they are entitled and ensuring services are delivered sensitively and respectfully
  • ensuring homeless clients who access housing are provided with the items they need to establish their tenancy
  • organising home care, shopping, meals-on-wheels and recreational support as required
  • finding out about aged care services and pension entitlements available through Veterans’ Affairs or Centrelink
  • referring clients into residential aged care or respite care if required
  • assistance in breaking down isolation through linking clients into community groups, recreational activities or arranging transport to chosen events. 

 

How much does it cost?

There is no cost to access Wintringham’s Outreach programs. The Outreach services that Wintringham provides are funded by a variety of Commonwealth and State Government programs.

Some services or programs to which Outreach staff may refer clients may have a cost, which would be explained to potential clients. These services may include: Home and Community Care Services, meals on wheels, rent and Community Aged Care Package fees.

Care finder and outreach support

How we can help you:

> Navigate the aged care and NDIS (National Disability Insurance Scheme) government systems.

> Sign up with a Home Care Package or NDIS provider of your choice.

> Support to apply for housing.

> Connect with support services in your community, such as health, mental health, housing and homelessness, drug and alcohol services, social and community groups.

 

Who is eligible?

Do you meet any of the following criteria?

> Aged 50 years+ (or aged 45+ for Aboriginal and Torres Strait Islander people).

> Need help finding or connecting with services and aged care supports.

> Have difficulty communicating because of language or literacy problems.

> Are in an unsafe situation or vulnerable if you do not receive services.
