Support at Home

We bring assistance to you at home.

What is the Support at Home program?

Support at Home is a new national aged care program replacing the Home Care Packages Program and Short-Term Restorative Care. It is a tailored service designed to help you stay at home, living independently for longer. It provides older Australians with coordinated, flexible support to stay safe in their own homes and connected to their communities.

Wintringham’s services operate under the Aged Care Quality Standards 2025, ensuring your rights and quality standards are central to everything we do.

Together with you, and your supporting team we work to understand their goals, interests and preferences, and support you to make informed choices about the services and assistance you need.

Support at Home program includes:

  • Eight support levels with funding up to $78,000 yearly, plus two short-term pathways.
  • Clear service list so you can know exactly what support is available to you.
  • 10% capped care management fee ensuring you receive trusted, value for money services.

To qualify for a Support at Home package you must first be assessed by the Commonwealth Aged Care Assessment Service. For more information about Support at Home, visit the Australian Government website: www.health.gov.au/our-work/support-at-home

What assistance can I receive?

Under the Support at Home program, there are three types of services you can receive.

Clinical Support

Personal medical and mental health services to support your health needs.

  • Nursing/clinical care
  • Allied Health
  • Nutrition
  • Care management
  • Social work

Independence Support

Assistance like personal care, social support or transport to keep you engaged in your community.

  • Personal care

  • Social support and recreational activities

  • Therapeutic services for independent living

  • Respite care

  • Transport

Everyday Living

Maintaining your home and daily life through cleaning, gardening or meals delivered.

  • Domestic assistance

  • Shopping

  • Essential home maintenance and repairs

  • Meal preparation

We will help you identify which services fit within your approved budget and link you to trusted support workers or if required third-party providers.

Our goal is to help you stay as independent as possible, with the right help, when you need it.

Additional short-term supports

There are various types of short-term supports available for when you need extra help for a short period.

  • Assistive Technology/Home Modifications (AT-HM) Scheme: This scheme gives eligible participants access to assistive technology and home modifications without needing to save from their individual budgets.
  • Restorative Care: Focused on helping you regain strength and independence through allied health services. Support is available for up to 12 weeks, with a possible 4-week extension, based on assessment.
  • End-of-Life Care: Provides additional in-home support for those with a prognosis of three months or less to live, aiming to help you remain at home for as long as possible. Funding of up to $25,000 is available, with services accessible for up to 16 weeks.

Once you're approved

Once you have completed the Government assessment, and you have received an approved Support at Home package, Wintringham will work in partnership with you to guide you through the Government-allocated budget and fee structure, so you can create the right care plan that not only suits your needs, but fits within your budget too. 

Our team will help you:

  • Develop your individual Care Plan and budget.
  • Explain how your contributions and funding work.
  • Ensure your plan promotes wellness and reablement — helping you regain skills and confidence to live independently.

All services begin with a Service Agreement that outlines what you can expect, how to change or cease services, and your rights under the new Aged Care Act 2024.

Who provides our care?

Wintringham provides all its clients with a choice of highly qualified staff with extensive experience in the provision of care services. Wintringham employs its own staff to ensure a good personal rapport and understanding of client needs is developed, which leads to a much higher standard of care.

Fees

As noted above, all package holders will be required to pay the 10% care management fee.

It is also important to note that for all new clients, and those approved for a Home Care Package or Support at Home package after 12 September 2024, you will be required to also pay co-contribution fees for your care services. Percentages vary depending on the service type. If you are an income tested fee paying client, this percentage will be determined by Services Australia (Government).

For example, if you are a full pensioner you will be required to pay:
Clinical support - 0%
Independence services = 5%
Everyday living services = 17.5%
